When you contact us, please provide the following information so we can deal with your complaint effectively:
* The reason for attending one of our centres or speaking with our clinicians at the call centre.
* The date and time of the appointment
* The staff members that deal with you
* What happened.
* What measures were taken at the time to raise and then solve the problem.
* How you would like to see the problem resolved and what changes you would like to see.
A member of the Quality and Compliance team will contact you within 3 working days of your complaint being received.
